Care Manager

PLAN of Pennsylvania provides Planning, Trust, and Care Management Services to families of adults with mental illness, intellectual disability, autism, brain injury, and other lifelong disabilities. PLAN uses a strength-based approach to maximize the potential of our clients, resulting in peace of mind for both them and their families. PLAN was founded by families to answer the important question, “Who will care for my loved one when I am no longer able?” PLAN was founded in 1989 and serves clients throughout Southeastern Pennsylvania.


The Care Manager is responsible for assisting those we serve in achieving their goals. We organize these goals under 6 major Life Areas (Benefits, Housing, Vocational, Health & Wellness, Personal Finance and Social Connection). Care Managers are able to engage the person, their support system and the human services community to assist the person in achieving the identified goals. Sometimes those we serve have a Special Needs Trust that can be utilized for the sole benefit, often financing Care Management and other much needed supplemental resources. This position requires travel within the Greater Philadelphia region (Bucks, Chester, Delaware, Montgomery, and Philadelphia counties).

Position Role & Outcomes:

  • Initial plan development and ongoing annual plan review with the person served and his/her support system.
  • Provide consultative review to direct families towards benefits, resources and services that will help their family member with a special need live a more meaningful life.
  • Work with the client on a continual basis in accordance with the guidelines and objectives of the PLAN service contract.
  • Utilize all available resources; private and county, government (etc.) to support the person served as necessary to maximize his/her well-being. (Including identification and referral to appropriate resources; crisis management; assistance with daily living activities; secure housing, public benefits, necessary medical care, home visits or transporting to and from appointments.)
  • Demonstrate capacity to assist with public benefits acquisition.
  • Serve as client advocate with support of PLAN of PA team.
  • Interact effectively within the PLAN of PA team to ensure that the financial and other support needs of the client are addressed.
  • Accurately and timely complete all of your own client engagement, documentation, billing and expenses.
  • Review and approve monthly billing to assure accuracy.
  • Collaborate with the PLAN of PA team to assist in improving family dynamics and addressing legal issues.
  • Attend continuing education classes or other trainings to continually develop industry and clinical knowledge.
  • Utilize effective problem solving skills to ensure the best/optimal outcome for the client.
  • Assist client in establishing and following a realistic monthly budget.
  • Other duties as assigned.


  • Minimum of Bachelor’s degree in Social Work while pursuing a master’s degree; MSW, LSW preferred.
  • Minimum of 2 years Case Management experience in mental health related field.
  • Excellent planning, organizational and interpersonal skills.
  • Some experience of government benefits, Social Security Administration, Waiver Program, Medicare, Medicaid, Section 8 Housing, SNAP, etc.
  • Excellent communication and writing skills.
  • Experience with Microsoft Office products and other software.
  • Understanding of budgeting and money management.
  • Valid PA driver license, personal transportation, and proof of insurance.
  • Requires valid criminal and child abuse clearances.


Compensation negotiable based upon experience; within a range of $45-$50K with benefits for a masters level LCSW candidate.
