How does someone sign up for case management services?

If you are interested in our case management services please contact our office and speak to a staff member. Call us at 610-687-4036.

 

Is there a minimum amount of service I need to sign up for?

There is no minimum amount of service that you must accept. PLAN of PA charges fees for both its Care Management and Trust Services. We do not accept insurance and, as a private social services agency, do not receive government support.

 

Does participation in PLAN of PA affect benefits?

Participation in PLAN of PA does not affect benefits. In fact, the principal goal of PLAN of PA’s Special Needs Trust services is to hold assets for the beneficiary without disqualifying him/her from receiving publicly-funded benefits, such as Medicaid and SSI (Supplemental Security Income.)

 

Is PLAN of PA a 24-hour service provider?

PLAN of PA staff are available Monday thru Friday from 9:00 a.m. to 5:00 p.m. unless otherwise arranged.

 

Does PLAN of PA serve as guardian for individuals with mental illness?

PLAN of PA does not serve as guardian but can serve as limited power of attorney for specific services.

 

Does PLAN of PA provide housing?

PLAN of PA does not provide housing; however our staff will work closely with the client to assist in finding housing for them.

 

What is PLAN of PA’s service area?

Our service area includes the greater Philadelphia area.

 

Does PLAN of PA take insurance? 

Not currently.
