Services

PLAN of PA offers several different services to meet the needs of our constituency. These services are:

Fee-for-service Case Management: Private case managers available to assure continuity of care and provide a level of service superior to that which the county provides. Case managers provide assistance in the coordination of services, linkages with vital resources, monitoring of progress, and ongoing evaluation and assessment. Each client’s services are specifically tailored to meet his/ her individual needs. Case managers schedule periodic visits to clients, listens to their concerns and those of their families, and maintains a flexible approach to the client's needs with a focus on individual strengths. Examples of other services in which PLAN can assist clients are:

  •  Helps to obtain public assistance such as Social Security benefits, food stamps, and Medicaid coverage.
  • Assists clients in locating housing.
  • Maintain contact with and coordinate services with social workers, psychiatrists, other medical and hospital personnel, dentists, county case managers, attorneys, trustees, banks, and other service providers.
  • Facilitate arrangements for vocational training and other educational services.
  • Assist with crisis management.

Representative Payee Service: PLAN can serve as representative payee, receiving benefits directly from Social Security and then dispersing them to, or on behalf of, clients in a clearly defined payment schedule. PLAN maintains individual checking accounts for each of its clients and reconciles their accounts on a monthly basis.

Special Needs Trust Services: PLAN can make available a professionally-managed trust that has been established specifically for PLAN clients to protect their benefits and prevent loss-of-funds as well as hold funds for the future. Initial contribution to open a PLAN trust account is a minimum of $5000; additional contributions may be made in increments of $1000 or more.
