Fee Schedule

Personal Care Plan Development:

$500 plus any travel time and mileage

Plans are developed that identify present and future support needs and the approaches that families desire for the care and security of their loved ones. A comprehensive Personal Care Plan contains vital information about the individual’s medical, residential, social, employment, educational situation and history. Plans are updated annually during the Annual Review.

Annual Review:

$150.

Annual reviews are required for all Case Management and Trust clients with a fee of $150.

Current Service Contract:

$100 for a 6 month contract (plus any travel time and
mileage that may be incurred.)

The $100 may be applied to the cost of development of a Personal Care Plan at the end of the six month period.

Consultation:

$125/hour

 

The Executive Director and Clinical Services Director are available to serve as a consultant for families or individuals with specific questions or concerns about future care planning but who are not seeking or not ready for a Personal Care Plan or a Current Service Contract.

Care Management and Trust Administration Time:

$105/hour

Charges based on the actual time spent providing services and includes face-to-face sessions, all phone contacts and certain forms and paperwork. Time is recorded in 12 minute increments.

Travel Time:

$52.50/hour plus $.51/mile

When services are performed outside of the PLAN of PA office, there is a charge for travel time.

Special Needs Trust Fee:

$500 minimum yearly fee or
1.5% of Trust balance up to $499,999
1.25% on the additional $500,000 -$999,999
1% on additional assets over $1,000,000
The fee is taken quarterly.

Up to 1% investment fee charged by Haverford Trust for trusts up to $2,000,000; .75% on the next $3,000,000.

Special Needs Trust Tax Return Preparation:

$135 minimum annually

This charge is based on the complexity of the required filings.

Special Needs Tax Payments (Federal and/or State):  

Amount due is based on the yearly Trust earnings and tax laws.

Representative Payee

$37/month

 

The Social Security Administration allows PLAN of PA, as an Organizational Representative Payee, to deduct $37 per month from each client’s SSI or SSDI monthly check.

Fiduciary Account Fee

$60/month

Active Case Management clients/client families who need PLAN of PA to pay bills and/or expenses can establish a fiduciary account that is managed by PLAN.
