Who can sign up for the Rep Payee program?
PLAN can serve as representative payee for adults (18+) with mental disability in our 5-county service area who receive benefits from the Social Security Administration.
What services does PLAN of PA provide through its Rep Payee program?
As Representative Payee, PLAN of PA arranges for the client’s Social Security check to be deposited directly into an individual, interest-bearing checking account, assures prompt payment of bills for basic food, shelter and medical needs and then helps develop a realistic, individualized budget for any remaining funds.
Do I have to have a case manager to sign up for the Rep Payee Program?
No, you do not need to have a case manager to participate in PLAN of PA’s representative payee program.
Is there a fee for Representative Payee services?
The Social Security Administration allows PLAN of PA, as an organizational representative payee, to deduct $37 per month from each client’s SSI or SSD monthly check. The remaining $38 that it costs PLAN of PA, per month, per client to run the program comes from donations, grants from private foundations and alternative sources of funding.

Representative Payee Services
Martha is in her 50's and struggles with both mental illness and medical issues. Martha's elderly mother, who has acted for many years as her Representative Payee managing all her finances, is no longer able to assume the responsibility.
PLAN of PA was contacted by the family and worked with family members and Martha to establish a budget, and set up an individual, interest-bearing checking account. Now her rent, utilities and medical bills are being paid in a prompt manner.
PLAN of PA is also working with Martha's other social service providers to ensure that her needs are being taken care of and to coordinate her care.
PLAN of PA serves as an Organizational Representative Payee for mentally disabled adults (18+ years of age) receiving SSI & SSD benefits from the Social Security Administration. PLAN of PA does not require that an individual have a case manager to become a client. How do I sign up for
Representative Payee
services with PLAN of PA?
To begin the process of naming PLAN of PA as your designated Organizational Representative Payee you must fill out the following forms and return the signed forms -- not copies -- to the PLAN of PA office:
- Application for Representative Payee Services
- Creating a budget for a new Representative Payee Client
- Request for Funds from Checking Account
- SSA Form 4164. This form must be filled out naming PLAN of PA as your designated Organizational Representative Payee.
- SSA Form 787 Physician's/Medical Officer's Statement of Patient's Capability to Manage Benefits. (This form only needs to be filled out if you do not currently have a designated Representative Payee.)
