The Care Assist program donates focused Care Management support to community members who would not otherwise benefit from these services. While there may be county resources available to them, many are either not yet connected or have been denied. Further, county funded agencies are overwhelmed and often the more challenging situations result in those individuals falling through the cracks.

For instance — there are many individuals who are eligible to receive government assistance, but don’t have the support to know what benefits exist, comply with the cumbersome application process and verify their need for assistance.

Enter PLAN of PA. We are experts in navigating the government assistance landscape. We know what documentation is needed, how to fill out applications, and know the right people to talk to in order to obtain the benefits needed to be a productive member of the community.

The most common ways Care Assist helps our friends with different abilities are obtaining healthcare, securing housing and resources for basic needs, and assembling a comprehensive medical team, just to name a few.

Anna’s Story

  • PLAN of PA first visited Anna at the referral of Joan, one of Anna’s friends who we also support. Joan noticed that Anna was getting sick again and wanted our help to advocate for her to get a health assessment. Joan explained that when Anna gets sick she tends to fall and Joan suspected that there was a recurring untreated illness.

  • Through our team’s advocacy, Anna was hospitalized to successfully treat Pneumonia, Acute Respiratory Failure and Sepsis. The hospital attempted to discharge Anna back to her apartment, but our team knew she would not have the supports needed there. Instead we advocated for a step down treatment first. This provided supervised treatment over the weekend until PLAN of PA could set up supports for Anna to return to her apartment.

  • When you give to Care Assist, your dollars are used to pay for short term targeted interventions that achieve real outcomes. Imagine Anna’s situation without the love of her friend and the advocacy of PLAN of PA. Any one of the three medical diagnoses may have resulted in a dire situation. Instead, Anna was supported to live a meaningful life.

Tony’s Story

  • PLAN of PA met Tony when his sister requested help. Tony is a responsible and likable person who lives with a moderate mental health diagnosis. Throughout his life Tony’s diagnosis has made it challenging to date, have friends and consistently maintain a job. Tony had some benefits in place, but they fell far short of his needs and his family asked PLAN of PA for help.

  • PLAN of PA worked with Tony to attain SSDI based on his work credit. We knew that gaining SSDI would change the healthcare options available to him. However, we were also aware that in 2 years Tony would qualify for Medicare through SSDI. By running a cost benefit analysis, we knew that gaining the SSDI was worth the loss of Medicaid. We were able to assist Tony in selecting a subsidized marketplace health care plan offering uninterrupted coverage with all his providers in network.

  • When you give to Care Assist your dollars are used to pay for short term targeted interventions that achieve real outcomes. As in Tony’s situation, our deep understanding of the intricate benefit system guides a strategy to gain the best combination of benefits. As a result, Tony gained substantial SSDI income based on his work credit. By predicting the loss of Medicaid, Tony had a marketplace solution in place with all his providers. This interim solution will remain in place until he qualifies for Medicare through SSDI. This success story demonstrates how Care Assist supported Tony to navigate his choices and advantage himself with the best combination of benefits.

Sara’s Story

  • PLAN of PA met Sara when she was 46 years old and living in Florida with her mother. Sara’s Mom was her primary support, assisting her with several challenging mental health and medical diagnoses. Sara loved her Mom, but also felt isolated as she only interacted with Mom and various treatment professionals. Sara and her brother contacted PLAN of PA after their mother received a terminal diagnosis. They wanted help moving Sara to Pennsylvania where her brother and his family lived. The requested for PLAN of PA to attain supported housing, move her benefits, and set up a new treatment team in Pennsylvania.

  • As Moms health declined, PLAN of PA opened a short term contract to deliver key outcomes. We were able to seamlessly transfer Sara’s benefits from Florida to Pa, establish a new treatment team and secure a Mental Health Personal Care Home within a reasonable distance to her brother’s home. Our team also obtained Sara a spot in a 5-day program that offers structure and engagement with her peers.

These outcomes help individuals such as Sara as they build more meaningful lives. As a result of this short term engagement, Sara now lives and works with her peers, has a solid treatment team and lives close to family.
