When to initiate Care Management?
Care Management can be utilized at many points throughout the lifetime. Services can be directed to assist as a brief intervention, or as a long term support. For long term support, services can be instated intermittently or continuously.

• 18-21: Transition from children and school services into the adult system.Change in functional status, requiring greater or lesser level of care.
• Transition into a facility or discharge from facility into community.
• Changes in family roles, need to establish new boundaries and norms.
• First episode of psychosis.
• Newly acquired/diagnosed disability.
• Change in financial situation.
• Family completing estate and end of life planning, want to document future care wishes.

How does someone sign up for case management services?

Is PLAN of PA a 24-hour service provider?
PLAN of PA staff are available Monday thru Friday from 9:00 a.m. to 5:00 p.m. unless otherwise arranged.

Does PLAN of PA serve as guardian for individuals with mental illness?
PLAN of PA does not serve as guardian or power of attorney. We can assist in determining if such services are needed. We can assist in locating and implementing such services.

Does PLAN of PA provide housing?
PLAN of PA does not operate a housing facility. However our team will work closely with the client and family to assist in finding appropriate housing.

What setting does PLAN provide service in?
PLAN does have an office space where meetings can be held. However, most often services are provided in the community. We travel to homes, doctors offices, facilities, schools, community settings, etc.

What is PLAN of PA’s service area?
Our service area includes the greater Philadelphia area. Generally this covers a 2 hour radius from Philadelphia and the Main Line.

Does PLAN of PA take insurance? 
We do not. PLAN of PA is a private pay service, intended to fill the gaps left behind by public benefits and insurance based services. This important service assists a person in maintaining autonomy while taking advantage of public benefits and insurance based services to the fullest capacity.

How do I pay for Care Management
We do not take insurance, waivers or state/government funding. A client can self-pay, but typically a friend, family member, or financial institution pays for services. Special Needs Trusts can pay for Care Management. We invoice monthly. We accept payment via check.

Does participation in PLAN of PA affect benefits?
Participation in PLAN of PA service offerings does not negatively affect benefits. In fact, the principal goal of PLAN of PA’s Special Needs Trust services is to hold assets for the beneficiary without disqualifying him/her from receiving publicly-funded benefits, such as
Medicaid and SSI (Supplemental Security Income).

Is there a minimum amount of service I need to sign up for?
There is no minimum amount of service that you must accept. PLAN of PA charges fees for both its Care Management and Trust Services.
