Our PLAN of PA Care Manager is a true Rock Star! She & PLAN of PA have worked with my niece and accomplished what we have all dreamed of for many years. My niece was struggling with multiple severe physical health diagnosis in addition to intellectual and psychiatric diagnoses. Further, she was in a very scary domestic violence situation where her safety was in jeopardy.
She has always wanted to live independently and there has been a team of professionals engaged to support her. There have been years of worry, hope, support as well as loads of patience. Even with that support, she was spiraling downward physically, emotionally, financially, and we were all very concerned.
I’m so pleased to say that PLAN of PA understood her situation and worked so well with the other agencies involved. You built a relationship based on trust and compassion. By so doing, you increased her confidence and self-esteem. Finally, you came to her rescue when she needed it most and facilitated her placement into an assisted living home. The maturity, tact, and empathy from our Care Manager was so impressive. She took hurdles in stride and exceeded expectations every step of the way. My family is so grateful. My niece is now living a life that has truly changed for the better – emotionally and physically. We look forward her future!
